Le mot "epai" a plusieurs significations, parmi lesquelles : colline et coupe. Cette grande randonnée de montagne (GR) tire son nom des deux.

D'une part, parce qu'il traverse plusieurs collines et, d'autre part, parce qu'à la frontière entre la Navarre et le Gipuzkoa se trouvent de nombreuses montagnes communales ou epaipide, d'où les habitants de la région obtenaient du bois de chauffage (epaitza ou epaice).

Entre Lizarrusti et Hondarribia, on traverse la réserve naturelle d'Aralar, le biotope protégé de Leitzaran et la réserve naturelle d'Aiako Harria. En plus de Nafarroa, le chemin passe également par Iparralde.
